If you are the owner of a declared dangerous dog, a restricted breed or a dog that is subject to control orders you must notify the General Manager of the new municipal area to which you are moving within 14 days.

I declare the above information to be true in every respect to the best of my knowledge and belief in accordance with Section 78 of the Dog Control Act 2000.
